MEDICAL CANNIBIS CERTIFICATION

Pacific Wellness & Integrative Family Psychiatry offers compassionate and legal certification evaluations for patients who qualify under Hawaii's Medical Cannabis Program (329 Card).

What is a 329 Card?

A 329 Card is a medical cannabis license issued by the Hawaii Department of Health that allows qualified patients to use medical cannabis for specific health conditions.

Who Qualifies in Hawaii?

You may be eligible for certification if you are 18 years or older and have one or more of the following qualifying conditions: IF you are a minor, please see the applying for minor form.

-Chronic pain

-PTSD

-Severe nausea

-Cancer

-Glaucoma

-Seizures and epilepsy

-HIV/AIDS

-Cachexia or wasting syndrome

-Multiple sclerosis

-Crohn's disease

-Rheumatoid arthritis

-Lupus

-Other debilitating conditions as approved

What We Offer:

-Medical evaluation by a licensed psychiatric nurse practitioner

-Guidance through the 329 Card application process

 -In-person certification visits

-Annual renewals and support via telehealth after initial certification

 How It Works:

-Schedule a Medical Cannabis Evaluation

-Complete an intake form at the DOH  https://medmj.ehawaii.gov/medmj/welcome

 and pay the state fee online

-Attend your in-person evaluation

We assist with your DOH registration and submission if you meet the qualifications.

Receive your 329 Card and instructions on safe use and access to the dispensary.
